One of the most consistent sources of anxiety for families receiving a home care referral is not knowing what happens next. When discharge planners can describe the assessment process clearly before families make the call, that first conversation goes far more smoothly for everyone involved.
At Advantage Private Home Care, the free in-home assessment is conducted by one of our patient care coordinators, who visits the home before care begins. The coordinator evaluates the client's daily routine, the physical environment, specific care needs, and the family's schedule and preferences. This is a listening conversation, not a sales call, and nothing is committed until the family is ready.
From the assessment, we build a written care plan that specifies exactly what each caregiver shift will cover. We then match the client to a state-verified CNA whose experience and availability align with that plan. The matching process considers both clinical fit and personal compatibility, because continuity of the same caregiver matters for both outcomes and family confidence.
For families coming out of a hospital or rehab stay, knowing these steps in advance reduces uncertainty that can delay a discharge. We serve 35 counties across Metro Atlanta and can typically begin care within 24 to 48 hours of the assessment. When referring a patient, what information do you find families most need before they're ready to engage with a home care agency?
