Most LTC policies do cover in-home care, including services provided by a CNA. Coverage typically begins once a physician certifies that your loved one needs help with two or more Activities of Daily Living (things like bathing, dressing, or mobility). There's usually an "elimination period" (think of it as a deductible measured in days rather than dollars) before the benefit kicks in, and policies set a daily or monthly maximum for what they'll reimburse.
