In home care, the power of prevention lies in proximity. Unlike episodic clinical visits, 24-hour and live-in caregiving creates continuous observation opportunities. Small changes in gait, appetite, mood, or medication tolerance are noticed because the same CNA is present day after day.
When caregivers are W-2 employees with RN supervision and continuity of assignment, those observations are documented and escalated. This is the difference between a near-miss and an early intervention. This is what prevents unnecessary hospitalizations.
For discharge planners and social workers: what preventive care gaps do you see most often when patients transition from clinical settings to home? We'd love to discuss how partnership models with stable home care teams can improve outcomes. #HomeCare #SeniorCare #PreventiveCare
